I've experienced four packet loss events in the last week or so, with up to 38% loss to the gateway and lasting up to 6 hours.
The last one I was at home (and awake) for happened Saturday (Feb 7) AM and was resolved after a modem power cycle (soft reboot of the modem had no effect), so I went and bought a new modem, a Zoom 5341J. Everything was great after that... until it wasn't. Overnight last night (Feb 10) I had another event lasting around four hours, with around 27% loss to the gateway most of that time.

Not sure this is relevant, but a few weeks ago, my average RTT dropped significantly, from around 10ms to maybe 2 or 3ms. A week ago, it abruptly went back up to around 10ms, accompanied by the first of the four packet loss events in the past week. I've attached a graph showing this.
