[AZ] Slow Speeds (Details Inside)

Modem: Arris Surfboard SB6190 (plugged in directly to wall - no surge protector) Router: ASUS RT-N66W (Tested speeds while bypassed) House Infrastructure: Straight cable to port in wall (no splitter or amp) Cox Ultimate Internet Tier *See below for day by day troubleshooting with Cox. I've been having a lot of speed slow downs, which has been going on for what seems like months now. The main problem is the upload speed drops to unbearable levels, choking my internet (can barely use voice/skype). Sometimes the speeds are perfect which makes the problem very hard to troubleshoot. Attached are speed tests done the past few days. As you can see the speeds are sometimes perfect for ultimate speeds (300+down, 30+up): [att=1] Here is the modem signal page during a slowdown period: [att=2] Day 1 (2/9/16): 1. Cox "Contractor" replaces all connectors. We discovered damage to the outside Cox box on the street (looks like a car tire hit it hard). Speeds were good before and after he did all the work. He stated he would put in for a maintenance call on the outside box to be repaired (he never did this). 2. Later that night the speeds were bad again so I called Cox to let them know again of slow speeds. This time they dispatched a COX in-house Tech the next day. Day 2 (2/10/16): 1. Cox In-house tech arrives. I demonstrated the slow speeds on 2 different computers directly connected to the modem. 2. He replaced a ground fitting at my COX box and proceeded to Test my line for ingress from the COX box on the street. My house infrastructure tested perfect. he also replaced the filter. Speeds continued to be slow. Day 3 (2/11/16): 1. The same COX in-house tech tested the line using his own test laptop and modem while I was at work. Unfortunately the speeds miraculously were good during his test. 2. We spoke on the phone when I got home at which point I ran a speed test which was also good (5 pm). He said that if my modem was also getting good speeds at this time it was most likely a plant (or tap?) issue. He stated he would dispatch maintenance which can take up to 48 hours. 3. Now @ 10pm the speeds are back to very slow. This is were I am now - I will keep updating this tread as I keep troubleshooting this issue with Cox. I'm not going to give up until this issue is fixed! What do you guys think the issue is (modem? plant? something else?). Let me know if I can provide anymore information that could help the troubleshoot process.
